Hospice referral information

Anyone can make a referral: A doctor, hospital staff member, nursing home, patient, or family member. Once the initial referral has been made, a doctor must sign an order stating that the patient is hospice-appropriate.

Once the order has been signed, a social worker and nurse will visit the patient and their family, admit the patient, explain hospice, develop a plan of care with the patient and their family, and complete paperwork.
