Palliative care is specialized medical care focused on relief of pain, symptoms and stress due to serious illness, whatever the diagnosis may be. It is an option for any hospitalized patient, of any age, who is living with a serious illness or facing complex medical decisions. It is best when introduced early in the care process and can be provided at the same time patients are undergoing treatment. By providing physical, emotional and spiritual support, we help patients and their families live the best possible quality of life through all stages of illness.
We can also help with:
- Navigating the health care system
- Guiding you through difficult treatment choices
- Coordinating care and the care team, including home care services, if needed
- Referring you to community support services after a hospital stay or as your condition changes
