When CYP3A4 is inhibited, statin levels in the bloodstream increase dramatically, potentially reaching toxic levels even at normal doses.
This can cause severe muscle pain, breakdown of muscle tissue (rhabdomyolysis), liver injury, and other complications. For older adults on statins considering supplementation: discuss all supplements with healthcare providers before starting; avoid grapefruit juice and fresh grapefruit; avoid red yeast rice (which contains statins); avoid high-dose niacin; consider CoQ10 supplementation (particularly ubiquinol form, 100-200 mg daily) if experiencing muscle symptoms, though discuss with provider first; separate fiber supplements from statin intake by several hours; maintain adequate vitamin D and vitamin K intake; monitor for muscle pain or weakness, reporting these to healthcare providers; and maintain regular follow-up with healthcare providers to assess statin effectiveness and side effects.
Muscle Pain and Statin Management
