Medication-Induced Nutrient Depletions
Metformin interferes with B12 absorption, causing deficiency in 10-30% of long-term users. This can lead to neuropathy (nerve damage) that may be confused with or compound diabetic neuropathy. Regular B12 monitoring and supplementation (via injection or oral high-dose supplemental cobalamin, which doesn't require intrinsic factor) is recommended.
Metformin also depletes folate, which plays important roles in homocysteine metabolism and is necessary for cell division. Low folate increases cardiovascular risk and may affect cognitive function.

Older adults with diabetes face particular challenges: they may not recognize or report hypoglycemia symptoms (older adults sometimes don't feel shaky or anxious when blood sugar is low), cognitive impairment may make diabetes self-management difficult, multiple medications increase interaction risk, and reduced kidney function (common in older adults with diabetes) affects medication and supplement handling.
Additionally, tight blood sugar control (aiming for very low HbA1c targets) increases hypoglycemia risk in older adults more than in younger individuals, and hypoglycemia episodes in older adults are more dangerous (increased fall risk, cardiovascular stress, cognitive dysfunction).
Managing Blood Sugar Safely With Supplements
For older adults with diabetes considering supplementation: discuss all supplements with healthcare providers before starting; report any blood sugar supplements or diabetes-relevant supplements (like magnesium or cinnamon); maintain regular blood sugar monitoring if supplementing; watch for hypoglycemia symptoms (trembling, sweating, rapid heartbeat, confusion, difficulty concentrating); ensure adequate nutrition and caloric intake (very low-carbohydrate supplementation without caloric compensation may increase hypoglycemia risk); and maintain regular follow-up visits with healthcare providers to monitor blood sugar control and medication effectiveness.
Recognizing Dangerous Low Blood Sugar
Caregivers should be alert to signs of hypoglycemia: shakiness, sweating, rapid heartbeat, confusion, difficulty concentrating, irritability, or behavioral changes. Older adults may be less aware of these symptoms or may attribute them to other causes. If hypoglycemia is suspected, testing blood sugar or consuming fast-acting carbohydrate (juice, glucose tablets) while awaiting testing is appropriate. Severe hypoglycemia (with altered consciousness or seizures) requires emergency care.
Practical Supplement Management in Diabetes
The safest approach for supplements in those on diabetes medications is: minimize supplementation, prioritize diet-based approaches to blood sugar control, consult healthcare providers before starting supplements, monitor blood sugar more frequently when adding new supplements, and keep a record of all supplements and any changes in blood sugar patterns to share with healthcare providers.
